How to read your Explanation of Benefit (EOB) statement

Verify the provider and patient names, dates of service and service descriptions. Look at the charges submitted to BlueLink TPA. In the following columns you’ll see:

  • Provider responsibility (the amount providers agree to accept as payment as part of their network contract)
  • Non-covered (items or services not covered under your benefit plan)
  • Allowed amount (the final amount to be paid by you and your benefit plan)
  • Any deductibles, co-payments or coinsurance that apply to this claim

Then, review the box on the bottom left side of the page. It lists:

  • Total charges
  • Amount paid by other insurance (i.e., if the patient is covered under more than one benefit plan)
  • Adjustment (any changes to the original amount paid due to new or additional information)
  • Total you owe (this is the amount for which you are responsible)
  • Provider responsibility (the amount providers agree to pay as part of their network contract)
  • Other adjustments
  • Total payment (the amount paid by your employer)
